This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Jennifer Miller Physical Therapy Services LLC, doing business as Move As You Should Physical Therapy (“we,” “us”), is required by federal law to protect the privacy of your protected health information (PHI). PHI is health information that can identify you, whether it is spoken, written, or electronic. This notice describes our privacy practices and your rights.
Our responsibilities
We are required to:

- Health care operations. Running the practice, including quality improvement, record-keeping, legal and accounting services, and business management.
- Appointment reminders and health information. Contacting you about appointments, treatment options, or health-related services we offer.
We may also use or share PHI when required or permitted by law, including:
- As required by state or federal law

Uses that require your written authorization
Other uses and disclosures, including most marketing and any sale of your PHI, require your written permission. You may revoke an authorization in writing at any time, except to the extent we have already relied on it. Minnesota law may require your consent for some disclosures that federal law would otherwise allow; we follow whichever law is more protective.

- Restrictions. You may ask us to limit how we use or share your PHI. We are not required to agree, except that if you pay in full out of pocket, you may ask us not to share that information with your health plan and we will agree unless the law requires otherwise. If we agree to a restriction, we will honor it.
